At Harmony Health, our mission is to serve as a community-centered provider of services and to improve the health, education and economic opportunities for individuals and families.  A non-profit organization with countless resources and services, we provide a holistic approach to creating happy, healthy families in our community.

 

What makes Harmony Health unique is our wide range of human services.   Our full-service family practice medical clinic provides treatment for acute illness and cradle to the grave primary care.  Our medical clinic provides prenatal care and is the only Comprehensive Perinatal Services Program in Yuba County.  Let our Registered Dietician, Licensed Clinical Social Worker, Midwives, RN and Internationally Board Certified Lactation Consultant, guide you through the most important months of your life - those leading up to the birth of your child.

Our integrated Behavioral Health services include Medicated Assisted Treatment (MAT) for Opioid Use Disorder and pain management modalities including Cognitive Behavioral Therapy, Bio/Neuro Feedback, Acupuncture, Chiropractic services, meditation classes and substance abuse support groups.

Harmony Health utilizes the PCMH concept which encourages you, the patient, to take an active part in your health care.  You will choose your personal clinician/care team, which includes medical and support staff.  You and your care team will be able to optimize and enhance your health through self-management tools, care plans and community resources.  You will have 24-hour access to the care team via scheduled appointments, walk-in/same day appointments and telephone advice/triage.

After hours, our answering service will connect you with the on-call provider to assess your situation and advise you accordingly.  Call 530-743-6888 during the day, night or weekend for assistance.
