Harmony Health is a Federally Qualified Health Center (FQHC). FQHCs include all organizations receiving grants under Section 330 of the Public Health Service Act. FQHCs qualify for enhanced reimbursement from Medicare and Medicaid, as well as receive other benefits. FQHCs must serve an underserved area or population, offer a sliding fee scale, provide comprehensive services, have an ongoing quality assurance program, and have a governing board of directors.

Centered Medical Home (PCMH) model, a progressive approach to healthcare delivery that

You will have the opportunity to choose your Primary Care clinician. Additionally, a dedicated team of support staff will collaborate with you to address all your healthcare needs. We will provide access to community resources, self-management tools, and personalized care plans designed to optimize and sustain your health.
Improved Tracking and Care Coordination:
After your discharge from the hospital or emergency room, our healthcare team will promptly schedule a follow-up appointment with you. This appointment serves to monitor your recovery progress and helps to prevent any unnecessary hospital readmissions. If needed, we'll identify, recommend and refer you to appropriate specialists for your care.
You can expect to receive your test and lab results in a timely manner. Depending on these
results, we may schedule an additional appointment to ensure you fully understand the
outcomes and any necessary follow-up steps.
Our team will also coordinate referrals to medical specialists and other healthcare services as required to optimize your overall health and well-being. We're committed to providing comprehensive care tailored to your specific needs.
Improved Health Access and Communication:
For urgent health concerns during business hours, you can count on same-day appointments with your Primary Care clinician or another team member. Simply call our main office number to schedule. After hours, our answering service will handle your call and connect you with an on-call provider. In case of a true emergency, always call 911 immediately. Remember, our medical health team can address many immediate health issues effectively. By contacting us first, you may avoid a lengthy and costly visit to the Emergency Room. We're here to provide prompt, efficient care tailored to your needs, whether during or after office hours.

You can reach out to a member of your Medical Home Team at any time during regular business hours. Our web-based system enables you to access information about specific medical conditions and receive electronic reminders for your appointments. We will provide you with additional details on how to enroll in MyChart.
How will I contact my Medical Home Team?
To schedule an appointment, simply call Harmony Health. For after-hours care, an on-call
provider will respond to your calls. However, for any emergencies, always dial 911 immediately.
It’s crucial that you or a designated representative inform your Medical Home Team if you
receive care from any other healthcare provider. This information allows us to effectively
coordinate all aspects of your healthcare, ensuring continuity and comprehensive management of your medical needs.
